The Sydney Advanced Gynaecology Endosurgery Centre (SAGE) is committed to providing quality health care for their patients. The SAGE Centre recognises the importance of ensuring that our patients are fully informed and involved in their healthcare.

The SAGE Centre is a health care provider in the private sector, bound by the Health Records and Information Privacy Act 2002 (NSW) and the Privacy Act 1988 this includes both the Australian Privacy Principles and the NSW Health Privacy Principles. These principles set the standards by which we handle personal information collected from our patients.

What kinds of personal information do we collect?

  • Personal details (name, address, date of birth, next of kin, Medicare number, DVA and other government identifiers)

  • Your medical, surgical and social history

  • Notes made during the course of medical consultations

  • Referrals to other health service providers, results and reports received from other health services providers

The information held about you is provided by you or arises as a consequence of information provided by you.

How do we obtain and file your personal information?

Electronic information is stored safely on the SAGE server. This information is confidential. Your file will be accessed in the following situations:

  • Consultations

  • Referrals

  • Other SAGE doctors (if your principal doctor is away) or by nursing staff as required

  • SAGE administrative staff for booking and billing purposes

SAGE staff are bound by strict confidentiality requirements as a condition of employment regarding your medical records. Your treatment may involve care at another health facility and by consenting to further treatment at these centres, it is implied that you have consented to your personal and medical information being sent to these centres. Information may also be conveyed to pathology companies, other medical practitioners involved in your care, medical insurance/indemnity organisations and fee collection agencies. Each centre involved in your care will have a Privacy Policy in accordance with legislation.

Can you access and correct your personal information?

The SAGE Centre advises that as a patient of the practice, you have rights of access to any information we hold concerning you.
